This text is the Kaddish Shalem (קדיש שלם) or Kaddish Titkabbal (קדיש תתקבל) – Literally "Complete Kaddish" or "Whole Kaddish."
Kaddish (קדיש, “holy") is a hymn of praise from the Jewish prayer service. The central theme of the Kaddish is the magnification and sanctification of G-d's name. Mourners "say Kaddish" to show that despite the loss they still praise G-d.

May his [G-d's] great name be exalted and sanctified in the world which He created according to His will may He establish his kingdom, during your days and during the lifetimes of all the House of Israel speedily and very soon, And say, So be it. May his great name be blessed forever, and to all eternity! Blessed and praised, glorified and exalted, honored, adored, and lauded, be the name of the Holy One, blessed be He. Above and beyond all the blessings, hymns, praises, and consolations that are uttered in the world. We say, So be it. May the prayers and supplications of all Israel be accepted by their Father who is in heaven, And say, So be it.
